Job Description - Nurse Practitioner-Internal Medicine-Physician Practice Full Time (3310488)
PURPOSE OF YOUR JOB DESCRIPTION:The purpose is to support the physician practices department and to provide coverage when the providers are out of the office.
DELEGATION OF AUTHORITY:They will report to the providers they work with and will utilize our nurses and MA’s in the offices as well.

PERFORMANCE REQUIREMENT / DUTIES AND RESPONSIBILITIES:
Diagnose and Treat acute and chronic Illness
Perform age-appropriate history and physical
Order and interpret diagnostic and therapeutic tests relative to patient’s age-specific needs
Prescribe appropriate pharmacologic and non-pharmacologic treatments to patient’s age specific need
Monitor the effectiveness of treatments
Provide education of acute and chronic medical conditions
Managing Chronic Medical Conditions
Reconciling medication list
Facilitating appointments to specialist
Providing thorough exams and evaluations of patient’s health status
Facilitate appropriate chronic care management (i.e. Diabetic eye exams, foot exams and nutritional counseling)
Preventive Medicine
Keeping track of and referring patient to appropriate preventive medical screenings (e.g. Mammograms and Colonoscopies)
Counseling and reinforcing patients in healthy lifestyle habits
EDUCATION REQUIREMENTS:• Primary Care NP certified in Massachusetts
EXPERIENCE:• 3 years experience in a Primary Care setting• Proficient Computer Skills (MS Word)• EMR experience required• Ability to document in electronic health record completely and accurately• Knowledge of clinical reminders and how to use them appropriately• Good communication skills, both spoken and written• Willing and able to work in a team environment• Able to change with the ever evolving healthcare field• Master Degree, ACLS, and PALS preferred
