Position Overview:We're seeking a compassionate and experienced LVN, RN or care manager to join our leadership team at Assisting Hands. The Care Manager will work directly with our company’s president, playing a pivotal role in overseeing the delivery of care solutions and services to our clients. The ideal candidate will be responsible for managing the delivery of services to our clients, supervising caregivers, and serving as a liaison between clients, caregivers, and other healthcare professionals. The Care Manager will be further involved in other aspects of the Company’s operations including sales and marketing and community relations.

Responsibilities include:
Conduct assessments of client needs and develop individualized care plans working with clients, their families, and healthcare professionals.
Coordinate care services based on client preferences and care requirements.
Recruit, train, supervise, and evaluate caregivers to ensure delivery of exceptional care in accordance with company policies and procedures.
Conduct home visits to assess client satisfaction, monitor the effectiveness of care, and address any identified concerns or issues.
Serve as a point of contact for clients and their families, providing support, guidance, and advocacy as needed.
Participate in community relations and sales and marketing activities with the Company’s owners and sales staff. Referral bonuses paid for new clients and caregivers.
Collaborate with healthcare providers, including physicians, nurses, and social workers, to coordinate and integrate care services.
Maintain accurate documentation of client assessments, care plans, and services in accordance with regulatory requirements and company standards.
Qualifications:
Minimum of 3 years’ experience as an LVN, RN and/or experienced homecare manager.
Strong interpersonal, communication, and leadership skills with the ability to collaborate with clients, caregivers, and other office team members.
Proficiency in computer skills, including Microsoft Office applications. Experience with care management and scheduling software preferred.
Compassionate, empathetic, and client-centered approach to care management.
Valid driver's license and reliable transportation.
