San Diego’s St. Paul's PACE program (Program of All-Inclusive Care for the Elderly).This innovative program is for individuals 55 years or older, who are living with chronic illness, or disabilities and need coordinated medical care to continue living as independently as possible in their home and community. The healthcare teams at St. Paul’s PACE provide a hands-on approach coordinating medical, social, and home care services so individuals no longer need to manage their medical care alone.Job Summary:
Under the direction on the In Home Services Supervisor, the IHS Staffing Coordinator is responsible for ensuring PACE participants are receiving their IHS services according to their plan of care (POC) by coordinating the schedules of IHS employees/caregivers. The coordinator will assign caregivers to PACE participants daily, organize efficient driving routes, ensure clear communication, and that the IHS employees/caregivers are following the assigned schedules and providing services as per the orders.
Duties and Responsibilities:
Attended Interdisciplinary Care Team (IDT) and plan of care (POC) daily meeting.Schedule IHS services for all PACE participants. Respond appropriately to needs as identified by the IDT.Coordinate IHS services for PACE participants so that they are timely and with highest level of operational efficiency.Prepare caregiver schedules and modify, cancel or reschedule as needed. Be able to adapt to unanticipated changes based on participant needs and IDT decisions, make adjustments, and communicate those changes to the appropriate affected personnel in a timely mannerReview and audit daily visits and prepare reports.Model and promote good customer service by maintaining positive relationships with PACE participants and their families, caregivers, colleagues and staff throughout the organization.Maintain adequate records for IHS services, and ensure established policies and procedures are communicated and followed.Report all issues promptly with possible solutions to Management.Perform other duties as assigned.Qualifications and Requirements:One year previous scheduling, administrative experience preferredPrior us of electronic medical records system (EMR), use of scheduling softwareAbility to work independentlyHigh School Diploma/GED required.Meet organizational driving requirements.Physical Requirements
: Requires manual and finger dexterity and eye-hand coordination, the ability to use department equipment and the ability to lift/carry up to 30 lbs. using appropriate body mechanics.Vision, Hearing, and Communication Requirements:
Requires corrected vision and hearing to normal range, with or without reasonable accommodation. Must be able to communicate verbally with all staff, vendors, and clients.Flexible paid time off (PTO) - starting at 28 days per year!Education reimbursement - up to $3,000 a year!Qualified Public Student Loan Forgiveness (PSLF) employer403(b) with matchChild care availableUpward mobility!You Make Us Who We Are!
