Job Summary
:
The Medical Director is responsible for the development, implementation, and oversight of PACE GNO primary care services.
Qualifications:Graduate of an accredited School of Medicine, Fellowship trained in Geriatrics preferred. At least 2-3 years clinical experience with supervisory and administrative experience. Able to supervise and direct MD's, Nurse Practitioners and other clinical professional staff. Must have a current license to practice medicine in Louisiana, a valid driver's license and reliable transportation. Must have proven clinical and administrative leadership abilities. Able to get along with others in a team environment. Ability to deliver service to physicians, patients and fellow employees with compassion in a responsive, courteous and concerned manner in accordance with PACE policies and procedures.
Essential Functions:
Responsible to oversee all medical care
Develop a strategic plan for the Medical Department that has goals and accountability
Participants Assessments and Care Plans - Initial, Semi-Annuals, and SICS
See participants for clinic assessment visits and acute care walk-ins
Provide Direct Participant Care
Interdisciplinary Team - IDT - Daily Meeting
Infection Control Oversight
Wound Care Oversight
Pharmacy Oversight - Medication Reconciliation and review
Manage and Prescribe Medication
ATM (Medication dispensing machine) in clinic requires MD agreement
Have a collaborating agreement with the nurse practitioners
Oversight of Medical Records documentation
ER visits, Hospital stays, Specialty appointment oversight and collaboration.
Utilization review
End of Life Consultation
Palliative care
Relationship Building - Providers, staff, hospitals, and community partners
CMS - Quarterly Calls and Audits Representation.
Quality Improvement - work with the PACE Quality Manager on Policies and Procedure
Attend PACE GNO Board Meetings
Attends or conferences into other Committee Meetings as needed
Performs human resource and operation management tasks for nurse practitioners
Collaborates with other PACE GNO leadership to ensure all clinical and health services are being met and carried out as discussed, ordered, and authorized
Serves as a liaison between the program and the physician community
Provides information and conducts presentations as required
Participates in other committees and work groups as needed
Keeping medical knowledge, experience, and licensing up to date
Adhere to all department policies and procedures
