WHAT YOU CAN EXPECT
Provide clinical oversight of patients reviewed by admission coordinators in conjunction with Physician to determine clinical readiness for IP Rehab admission. In conjunction with Physician, determine Medical Necessity with admission coordinators to comply with CMS requirement for Physiatry daily management.Is a resource for admission coordinators for questions regarding patient appropriateness for IP Rehab admission and compliance with CMS regulationsEnsure patients transition smoothly by completing and providing oversight of admission and financial responsibility forms, coordinating, and arranging physical, social, emotional, and support service requirements, including transportation.Ensure confirmation of insurance eligibility, copays and barriers to discharge prior to acceptance for admission.Communicate with patient and or family any copays or financial expenses known at the time. Facilitate discussions with financial counseling for self-pay or charity care application completion.Administrator of Allscripts application they utilize to receive and respond to external referrals; add people to app and removes when they leave, teach others how to utilize app, and is the contact person for the vendor.Prepare and provide informational brochures; perform marketing visits for relationship and referral cultivation; answer questions; and conduct tours to market programs and facilities.Review, analyze, and maintain admission guidelines to recommend changes to admission criteria and procedures.Collect, analyze, and summarize admission data and trends to prepare admission reports for the leadership team.Stay abreast of what competitors and referral sources are doing to take advantage of or respond to threats and opportunities for our Service Line.Process admittance application by comparing the patient's condition to admission criteria to make admittance decisions and to refer patients and family to other programs and institutions. Review findings with the Rehabilitation Physician to determine appropriateness for admission. Maintain compliance with all related Regulations for CMS and ACHC.Serve as liaison between admissions and colleagues; confer with team members regarding department needs re appropriate admissions; recommend changes or additions to current system to enhance effectiveness of department.Complete admission Patient Assessment Screen (PAS) for physician review and signature prior to admission. Communicate and coordinate determination and transition with the referring facility. Provide coordination of resources within the administrative department to ensure compliance.Investigate patient concerns and recommend changes in service policies and procedures in order to resolve and maintain patient and family satisfaction.Follow safety policies, procedures, and regulations to maintain patient safety.QUALIFICATIONS
Indiana Registered Nurse (RN)
