Empath Home Health, a member of Empath Health, is seeking a PRN Lymphedema Certified Physical Therapist PT- Home Health. The Home Health Physical Therapist treats the patient by use of mechanical means, such as exercise and heat, to return and maintain the patient to his/her optimum level of physical functioning.The Home Health PRN Lymphedema Certified PT - Physical Therapist:Provides physical therapy services to patient as prescribed by a physician in compliance with agency policies and procedures and federal and state regulations.Performs initial and ongoing assessments, including OASIS assessments at appropriate time points, to determine level of functioning and patient's ability to perform activities of daily living.Develops and revises a plan of care, in consultation with the physician and other care team members, which will enable patient to achieve realistic goals of mobility.Reports to the physician and designated agency representative patient's reaction to treatment and any changes in the patient's condition.Requirements:Licensure as a physical therapist in the state of FloridaMinimum of one (1) year experience in home health care preferred.CLT - Certified Lymphedema Therapist in the respective state.Suncoast Hospice, Empath Home Health, and Empath Health Pharmacy are proud to be accredited by the Joint Commission showing our commitment to quality.
