Position is responsible for increasing the utilization of clinical services offered by Memorial Hermann.

Minimum QualificationsEducation:

Graduate from an accredited school of professional nursing, vocational nursing or licensed practical nursingLicenses/CertificationsCurrent Texas RN license required; BSN, CRRN, CNRN or CCM preferredCertificate in Basic Life Support requiredAdditional certifications will be required based on discipline and/or nursing unit requirementsExperience / Knowledge / Skills:Minimum of three (3) years nursing experience in rehabilitation, ER, SICU/NICU nursing, or related fieldExperience in similar position or Case Management strongly preferredAbility to manage multi-task, prioritize work, pay close attention to details and meet deadlinesBecause of the education and training required to obtain licensure, demonstrate principles or care of pediatric and geriatric patients within the rehabilitation environmentExcellent organizational and communication skillsClinical knowledge to assess patients for hospital programsWorking knowledge of various data systemsAbility to effectively communicate complex ideas (both conceptual and concrete) by oral and written meansAbility to work independently with little or no supervision and demonstrate excellent problem solving and critical thinking skills when making decisionsPrincipal AccountabilitiesConducts clinical assessment of patient referrals and presents assessment to physicians for admission.Participates in ongoing process improvement efforts related to the admissions process.Serves as a resource to directors to identify processes that hinder referral source’s ability to us Memorial Hermann as their facility of choice.Demonstrates a complete and thorough understanding of programs and can effectively market services.Participates in hospital marketing programs, events, conferences or in-services.Interpersonal skills necessary in order to interact effectively with external referral sources, including social workers, discharge planners, case managers, other healthcare professionals, families and patients; Develops strong working relationships with external referral sources and internal staff including Admissions, physicians, Outpatient Clinic, Marketing staff and other departments; and ability to adhere to confidentiality rules.Performs duties in a manner to promote quality patient care and customer service/satisfaction, while promoting safety, cost efficiency, and a commitment to the CQI process.Ensures safe care to patients, staff and visitors; adheres to all Memorial Hermann policies, procedures, and standards within budgetary specifications including time management, supply management, productivity and quality of service.Promotes individual professional growth and development by meeting requirements for mandatory/continuing education and skills competency; supports department-based goals which contribute to the success of the organization; serves as preceptor, mentor and resource to less experienced staff.Demonstrates commitment to caring for every member of our community by creating compassionate and personalized experiences.
