JOB TITLE:
Optometrist - Part Time 21 hours/week
REPORTS TO:
Chief Medical Officer
The following statements reflect the general duties, responsibilities and competencies considered necessary to perform the essential functions of the job and should not be considered as a detailed description of all the work requirements of this position.
RESPONSIBILITIES:Provide outpatient care for patients with eye-related disorders and conditionsConduct routine eye examinations, including assessment of visual field testsDetermine each patient's visual acuity, field of vision and hand-eye coordinationTake detailed medical histories for all patients, including current and past prescription medicationsEvaluate eye-related symptoms, such as discharge, redness and inflammationPrescribe corrective lenses when requiredEducate patients on proper eye careIdentify eye health safety factors that could impact vision, such as working around chemicals or in front of a computerAdvise patients about proper eye hygiene and careAdheres to professional standards, practice policies and procedures, federal, state, and local requirements, and regulatory standards.Ability to diagnose and treat eye-related disorders and conditionWorks as a team player assisting other staff providers with developing diagnosis and patient treatment options.Orders, performs and interprets test and analyze records, reports and examination information to diagnose patients' condition.Obtain and maintain hospital privileges at Nassau University Medical Center in order to provide care for HHLI patients in the hospital setting.Records and maintains patient information such as medical history, reports and examination results.Monitors patients' conditions and progress and re-evaluation treatments as necessary.Reviews health risk assessments (HRA's) to help determine a plan of care.Refers patients to medical specialists or other practitioners when necessary.Participates in LEAN and other activities necessary to implement the multi-disciplinary patient-centered medical home approach for the care of patients.Updates job knowledge by participating in continuing medical educational opportunities; reading professional publications; maintaining personal networks; participating in professional organizations.Other duties as assigned.QUALIFICATIONS:
Graduated from an accredited optometry school and licensed to practice in New York State.Board certified or board eligible status.At least 1-2 years of experience preferred.Ability to travel to other sites for coverage as needed.Bilingual in English and Spanish or Creole/French preferred.This position is part-time, 21 hours per week, with potential to increase hours over time.
SALARY : Commensurate with experience
