Job Summary:
The RN Clinical Nurse Navigator is a nurse with a specific body of knowledge regarding the diagnosis and treatment of cancer. The job is responsible for the educating and navigating cancer patients through the JPS healthcare system and facilitates timely diagnosis through the treatment process.Essential Job Functions & Accountabilities:Demonstrates competencies in understanding the process of diagnosis and treatment of cancer and the assessment of patient and family educational and emotional needs. Acts as primary educator and support of patients and families followed in the Oncology and Infusion Clinic.Coordinates the development, implementation and evaluation of patient focused outcomes through Clinical Pathways and/or Plan of Care. Which includes medical, surgical, infusion, and radiation treatments. Participates in Tumor Conferences to understand the plan of care.Assesses patient's physical and psychosocial needs and makes referrals as appropriate. Coordinates patient care across the network and appropriate organizations by coordinating services with Social Services, Cancer Care Services, American Cancer Society and any other organization that would be of help to the patient by facilitating all necessary diagnostic processes by assisting with barriers such as lack of insurance, financial issues, need for personal support, language, and any other problems. Serves as a liaison.Documents patient care services, concerns/issues and/or other relevant information in the patient and department records and the computer system for nursing and medical review.Collaborates with the healthcare team to develop, implement, and evaluate educations materials provided to patients and families.Serves as nurse educator for other physician/nursing staff through in-service programs or one-on-one in understanding the different kinds of cancer diagnosis, their treatment, the patient's emotional and social recovery from the disease.The navigator is responsible for collecting outcome data related to the program and evaluating the effectiveness of the program. The navigator is also responsible for keeping a log of patients seen, services provided, studies log to prevent patients lost to follow-up.Delivers quality nursing care while minimizing the risk of infection, contamination, harm, or injury. Maintains safe and clean working environment ensuring compliance with procedure rules and regulations.Maintains accurate, detailed reports and records and directs care with regard to age and developmental stage, presenting complaint, and pertinent history.Serves as leader and facilitator of support group/survivor program.Modifies patient treatment plans as indicated by patients' responses. Monitors, records and reports symptoms and changes in patients' conditions.Job description is not an all-inclusive list of duties and may be subject to change with or without notice. Bachelor's Degree from an accredited school of professional nursing or obtain within 1 year of employment.2 plus years of experience in an oncology setting.Current licensure by the Board of Nurse Examiners for the State of Texas or proof of reciprocity of licensure between the State of Texas and another state.Completion of unit specific competency based orientation program within 90 days.Current JPS recognized Basic Life Support (BLS) certification and must be maintained throughout employment.Certification in one of the following from Oncology Nursing Certification Corporation (ONCC) or Academy of Oncology Nurse and Patient Navigators (AONN): Certified Breast Care Nurse (CBCN); Advanced Oncology Certified Clinical Nurse Specialist (AOCNS); Oncology Certified Nurse (OCN); Advanced Oncology Certified Nurse (AOCN); Oncology Nurse Navigator - certified generalist (ONN-CG); Oncology Patient Navigator - Certified generalist (OPN-CG); National Consortium of Breast Centers (NCBC): Breast Health Clinical Navigator (BHCN);if not at hire date, must obtain within 24 months of hire date.
Preferred Qualifications:
Master's degree.Previous Patient Navigation Experience.3 plus years of experience in Oncology.
