Overview
Responsible for the development, implementation and evaluation of systems and programs directed toward the provision of quality services for continued program accreditation. Provides diagnostic and therapeutic health care to patients under the supervision of a licensed physician, will work in collaboration with interdisciplinary system partners.Responsibilities
ESSENTIAL DUTIES AND RESPONSIBILITIESClinical Activities:* Serves as an expert practitioner, role model and clinical resource in providing quality nursing care to program specific patients and their families.* Support the care team in clinical management of the program specific patients.* Follows program specific patients from admission through discharge and transitions of care as specified by program requirements.* Assist in ensuring all care throughout continuum is coordinated, cost -efficient, and results in optimal outcomes.* Assist in monitoring compliance with program specific standards of care.* Coordinate multidisciplinary team services for patient and family needs.* Provide and document program specific care in accordance with standards relating to the care of these patients.* Identify the need for and coordinate the development and implementation of care paths for program- specific patients incorporating members from the multidisciplinary team.Administration:* Represent the Programs on various hospital and community committees to enhance and foster optimal patient care management.* Manages, coordinates and leads Program Steering and/or Operational meetings.* Manage, as appropriate, the operational, personnel and financial aspects of the Program.* Supervise adherence to hospital policies, procedures and standards through observation, medical record review, staff feedback and other appropriate sources.* Supervision of Program database.* Promote customer service.* Facilitates resolution of patient concerns and implements processes to prevent future incidents.* Ensures the Program meets all current evidence based guidelines and program specific standards as outlined by accrediting agency.Education Responsibilities:* Develop, coordinate and implement orientation, nursing education, and in-service programs related to care and management of Program specific patients in conjunction with nursing services.* Direct community program specific education and prevention programs by developing, implementing, and evaluating programs for targeted populations in the city and surrounding communities* Participate in mandatory educational activities as appropriate to professional development goals.* Participate in case review.* Participate in education programs as needed.* Review all present Program specific patient education materials and standardize across the system.* Work in collaboration with Program specific coordinators across the system* Maintains operations in all assigned areas in compliance with corporate compliance programs, JCAHO standards, IDPH regulations, and other applicable standards and regulations.CULTURE OF EXCELLENCE BEHAVIOR EXPECTATIONSTo perform the job successfully, an individual should demonstrate the following behavior expectations:Quality
