Job Summary & Responsibilities:
Responsible for supervision and operational direction of home health and or hospice operations. Provides supervision of the clinical care and non-clinical services delivered to patients with twenty-four-hour accountability for patient care and operations of the home health and or hospice agency. Performs duties in accordance with applicable state, federal and TJC regulations. Assumes responsibility to achieve performance improvement objectives, financial outcomes, and clinical outcomes to assure the organization meets the needs and expectations of its patients, families and referral sources. Assures collaboration across the enterprise and is a service excellence champion to ensure the agencies are constantly striving to be the provider of choice.

Associate Degree NursingBachelor required within 2 years of hire or transfer for Nurse candidates only.1 year of recent home health/hospice/palliative care.
