This therapist will provide occupational therapy services for children from birth through age three with disabilities or special developmental needs, helping them to build on their child's strengths, and to enhance and promote the child's growth, development, and learning skills in their natural environment. Therapy services are provided to infants and toddlers in the home, daycare, or community at the most critical time in a child's life, to address developmental delays in the areas of cognitive, adaptive behavior, social-emotional, self-care, fine motor, gross motor, and communication development. Utilizing a coaching model, caregivers are included in the teaching process, so therapy can carry over in all aspects of the child's routines and environments.

JOB SUMMARYFacilitates restoration of function and prevention of physical disabilities for patients with rehabilitation needs and/or physical disabilities by planning and administering medically prescribed occupational therapy.

ESSENTIAL FUNCTIONSIDENTIFIES PATIENT REHABILITATION CARE REQUIREMENTSBy assessing patient condition upon order of physician and at regular intervals during assigned shiftintegrating patient information to facilitate holistic patient care.

PLANS, IMPLEMENTS AND EVALUATES REHABILITATIVE CAREBy setting goals, collaborating and communicating with health-care team, patients and familiesacting on physician's order and patient need in a timely mannerdocumenting interventions and responsesrevising care plans based on patient response.

PROVIDES INFORMATION TO PATIENTS AND HEALTHCARE TEAMBy meeting themproviding educational information in conjunction with direct care providers related to treatments, procedures, and continuing care requirements; answering questions and requests.

MONITORS DELIVERY OF CAREBy completing patient roundsdocumenting careidentifying process improvements leading to desired care outcomesintervening to overcome deviations in the expected plan of carereviewing the plan with patients in conjunction with direct care providersmaintaining ongoing communication with care management regarding variances from the care plan or transfer/discharge plan.

EducationBachelor's Degree in Occupational Therapy required

Work Experience1 year of acute care experience preferred
