is seeking to hire a
full-time / Transitional Care Management Nurse (LPN)
to join the ACO team. We are looking for a new team member who is positive, energetic, well-organized, can multi-task, and think outside of the box. We want someone who supports our top initiative of ensuring an excellent patient experience!The
Transitional Care Management (TCM) Nurse
communicates with every patient that has been discharged from a hospital or Skilled Nursing Facility within 24 to 48 hours. S/he communicates with community services utilized by the patient. S/he identifies available community services and health resources and facilitates access to care and services available to patient/family. S/he provides education of patient and family members to support self-management, independent living and activities of daily living. S/he is responsible for documentation of responses and charting as needed in patient's EMR chart. S/he schedules TCM face to face visit with PCP. S/he provides assessment/support for treatment regimen adherence and medication management. S/he answers incoming phone calls, handles incoming and outgoing faxes, makes copies, and does various other office tasks as assigned.This position earns
