Rock Creek Care Center is proud to be a five-star Medicare-certified, short-term rehabilitation and skilled nursing home located in Auburn, California.Provide physical therapy interventions to help patients achieve their goals and improve their physical functioning.Utilize therapeutic exercises, manual therapy techniques, modalities, and equipment to enhance strength, mobility, balance, and coordination.Educate patients on exercises to be performed at home to facilitate continued improvement.Regularly assess and document patients' progress, adjusting treatment plans as necessary to ensure optimal outcomes.Maintain accurate and up-to-date patient records in compliance with regulatory standardsEducate patients and their families about the therapeutic process, goals, and expected outcomes.Provide guidance on adaptive equipment, assistive devices, and environmental modifications to facilitate independence and safety.Work closely with a multidisciplinary team, including occupational therapists, speech-language pathologists, doctors, and nurses, to provide holistic care.Participate in team meetings to discuss patient progress and treatment plans.CA Lic required1 year experience
