Responsibilities:

Overall responsibility for the assessment and treatment of all medical needs of Hospice & Palliative Care assigned patients. Performs preliminary assessments and consults in addition to on-going follow-up visits of the patient's and family's emotional, social, spiritual and environmental needs. Coordinates care so that patients or family members receive needed supplies, treatments and interventions to alleviate the physical, psychological, social and spiritual pain.

Qualifications:

Graduate from an accredited Medical College. Completion of post-graduate internship and residency required. Doctor of medicine or osteopathy with current license to practice and prescribe scheduled drugs. Minimum of two years of experience post residency program with familiarity of hospice and or palliative care. Hospice and Palliative Care board certification required or obtained within one year of hire. Staff privileges at participating healthcare facilities. Expertise in palliation of common symptoms of terminal illnesses. Good working knowledge of PC based applications.
