Cadre Hospice seeks to expand access to the hospice benefit, especially among underserved populations. In addition to traditional go-to-market channels, Cadre plans to partner with at-risk medical groups to integrate upstream and offer a comprehensive palliative care program to patients nearing end of life. Cadre combines a unique pre-hospice care model, proprietary patient identification algorithm, partnership mindset with provider groups and best-in-class hospice operations to offer the most comprehensive end-of-life care solutions to patients.
Key Responsibilities:
Leadership
an d Strategic Planning:
Provide visionary leadership to develop and implement strategic plans for the hospice program, in alignment with organizational goals and objectives.Foster a culture of excellence, collaboration, compliance, and innovation among staff to ensure the delivery of high-quality, patient-centered care.Identify growth opportunities and develop strategies to expand and enhance hospice services in the community.Incorporate continuous quality improvement principles into the day-to-day operations.
Clinical Operations Management:
Demonstrate a commitment to excellence through focusing on the highest standards of patient care.Oversee all clinical operations of the hospice program, including patient admissions, care planning, and coordination of interdisciplinary care teams.Ensure compliance with regulatory standards and accreditation requirements, including Medicare Conditions of Participation and state licensure regulations.Monitor and evaluate clinical outcomes and performance metrics to drive continuous improvement in patient care quality and satisfaction.
Staff Management and Development:
Recruit, hire, and onboard qualified clinical and administrative staff members, ensuring adequate staffing levels to meet patient care needs.Oversee/Provide core orientation for all team members, job-specific orientation, ongoing in-service training and education for employees, volunteers, and contractors.Provide leadership, supervision, and support to staff, fostering a culture of professional growth, accountability, and teamwork.Promote positive communications to keep employees informed of changes through frequent meetings, both team and individual, being conducted and documented.Conduct performance evaluations, provide constructive feedback, and facilitate opportunities for staff development and training.
Financial Management:
Develop and manage the hospice program budget, including revenue projections, expense monitoring, and financial reporting.Implement cost-effective strategies to optimize resource utilization and maximize revenue generation while maintaining high-quality patient care standards.Identify opportunities for revenue growth, including payer contracting, referral development, and fundraising initiatives.
Community Relations and Outreach:
Develop meaningful relationships with healthcare providers, community organizations, and other stakeholders to promote collaboration and enhance the visibility of hospice services.Participate in community events, educational activities, and marketing initiatives to raise awareness about hospice care and establish referral relationships.Represent the organization in local, state, and national forums to advocate for hospice patients and advance the mission of end-of-life care when needed.
Qualifications:
Current Registered Nurse (RN) license in good standing for the state of practice required.Bachelor's degree in Nursing (BSN) preferred and / or Master's degree in Business, Nursing Leadership, Healthcare Administration, or related field, preferred.Minimum of one year of experience with management/supervision in hospice home care setting required.Maintains current CPR certification.Minimum of three years of experience in hospice and/or palliative care setting preferred.Strong clinical knowledge and expertise in end-of-life care principles, pain management, and symptom control.Demonstrated proficiency in strategic planning, budget management, and quality improvement processes.Excellent interpersonal, communication, and problem-solving skills, with the ability to build and maintain effective relationships with diverse stakeholders.Knowledge of regulatory requirements and accreditation standards related to hospice care, including Medicare Conditions of Participation.Commitment to the organization's mission and values, with a passion for providing compassionate care to patients and families facing end-of-life issues.
