BAYADA Home Health Care is currently seeking an Outpatient Physical Therapist, PT, for a full-time OR part time opportunity performing outpatient visits in independent living/assisted living/memory care communities for our Senior Living office, located in Loveland, CO.
As an outpatient Physical Therapist, you will be an integral member of a multi-disciplinary health care continuum team that provides outpatient services to clients affording them the opportunity to age in place.

Responsibilities:
Make outpatient visits to clients in designated senior living communities in the Loveland area.
As necessary or applicable, supervision of Physical Therapy Assistants, in accordance with state licensure requirements and professional standards.
Attends and contributes to patient care, staffing conferences and other related meetings.
Makes referrals to other disciplines in order to address the comprehensive needs of the patient.
Perform diagnostic tests and measurements, such as the mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments.
Develop and implement appropriate individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
Continually assess and revise the Physical Therapy - PT plan of care, and participate in the multidisciplinary care plan meetings including community staff, as appropriate.
Educate and instruct clients, family members, or community staff, in therapy services and activities necessary to promote the client's health, safety, and independent living.
Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet.
Qualifications:
A current CO Physical Therapist - PT license.
Outpatient experience a plus but not required.
Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma.
Ability to work independently and manage time effectively.
Dynamic and strong interpersonal, organizational, and problem-solving skills.
Solid computer skills: prior experience with electronic medical records (EMR) preferred.

Additional Information:
Pay – Weekly, Starting at $40-$47 per hour, based on experience.
