Monday - Friday 8-4:30

No nights, no holidays and no weekends

Major Responsibilities: Provide safe transportation to and from client's home to the Adult Day Service program.

Position clients in seat and wheel chairs so that seat belts are used correctly, being courteous at all times. Obey all traffic laws by observing safety regulations and maintaining a safe driving record. Use knowledge, skill and good judgment in driving to avoid any action that would be injurious to clients.

Under the supervision of the activity staff assist in client programs

Transfer clients from one activity to the next by walking with them, pushing wheelchairs, directing dementia clients to correct activity areas. Help monitor the whereabouts of clients, being especially aware of those clients who tend to wander In conjunction with other staff, make sure clients have badges on Lead activity programs such as bingo, pokeno, card games, active games, etc be able to assist CNA/NCT's, RN's with personal care for participants as needed

Assist in the serving of food at lunch time and snack for AM and PM

Help serve food to clients during lunch time Assist individual clients by cutting up food, buttering bread and opening plastic ware. Clean off tables as needed (including washing of tables with antibiotic solution) Take and record temperatures of hot and cold food as directed by Site Manager Clean up in kitchen (washing pans, cleaning counter tops, placing dirty dishes in dishwasher and starting dishwasher

Inspect ADS bus vehicle daily to detect problems to be called to the attention of ADS Manager

Make sure windshield wipers, fluid, lights, brakes are in good working condition Check safety sticker to make sure it is current. Bring to the attention of ADS Manager when vehicle needs to be rechecked for safety inspection. Schedule routine safety inspections in conjunction with supervisor. Gas vehicle before or after routes making sure fuel is at appropriate level. 4)Schedule routine oil changes and other routine maintenance as needed with ADS manager; make sure ongoing maintenance is completed. Report any problems to Manager Schedule, take vehicle for regular wash. Clean interior of vehicle weekly or more frequently as needed. Include sweeping, wipe down of seats, and cleaning interior windows. Make sure vehicle's insurance card is current. Report any problems to supervisor. Make sure vehicle's city sticker is current. Report any problems to supervisor.

Establish daily route schedule in-conjunction with site supervisor to accommodate Adult Day Service participant's relying on bus transportation

Group participants according to area Make sure routes are updated as needed Change transportation schedules to reflect new routes Ensure clients are aware of all scheduled time changes for routes

Escort clients by providing door to door service from home to center and center to home.

Follow individualized instructions for each client Take client into their home (apartment) Assist client with removal of coat if necessary Make sure clients are "safe" before leaving Hold client by arm when escorting if needed Monitor safe walkways at client's home (be aware of snow and ice buildup, report to supervisor immediately.

Observe changes in clients' behavior and functioning levels. Report changes to supervisor immediately Participation in ongoing education

Attend in-services on safety, transferring techniques, working with dementia clients Attend CPR classes to maintain recertification or initial certification Know universal precautions in daily performance of procedures complete all CBT's as required

Education/Experience Required:

current Drivers License experience working with adults and or older adults with assisting them with ADL's Knowledge, Skills & Abilities Required:

Good driving record CPR Training on site CNA/NCT a plus Physical Requirements and Working Conditions:

Must be flexible Light to medium lifting needed Ability to work with frail older adults Needs to have patience when working with day service clients If position has direct patient care or direct patient contact the following lifting requirement supersedes any previous lifting requirement effective 06/01/2015. Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available. Unique patient lifting/movement situations will be assessed on a case-by-case basis.
