Work Schedule:100% FTE, 40 hours/week. 8 or 10-hour day shifts scheduled Monday through Thursday between the hours of 7:00 AM - 6:00 PM and Friday 7:00 AM - 5:30 PM. Physical Therapist
to:Evaluate and treat an orthopedic/spine chronic pain patient population of all ages, develop and guide this patient population through appropriate individualized rehabilitation programs.Efficiently communicate with physician referral sources, and other UW Health departments for care coordination and collaboration.Work within an interdisciplinary pain management team to continually provide exceptional patient and family centered care.

Qualifications
Bachelor's Degree in Physical Therapy Required
Work Experience
2 years of prior relevant experience Preferred
Licenses & Certifications
Licensure as a Physical Therapist in the State of Wisconsin Upon Hire RequiredBasic Life Support (BLS) certification within 3 months Required
