Job Summary:
Provides professional physical therapy services by evaluating, planning, implementing, and documenting patient treatments. Conducts assessments and develops personalized treatment plans based on physician orders to enhance functional abilities across all areas of a patient’s life. Adjusts treatment plans as needed throughout the care process. Works closely with Physical Therapist Assistants (PTAs), providing instructions and supervision for treatment procedures. Educates patients, families, and caregivers on exercises and therapeutic home programs to continue after discharge. Completes timely documentation and billing in accordance with hospital, rehabilitation department, Indiana State Practice Act, APTA, and Joint Commission standards.
Education:
Graduate of an accredited physical therapy program.
Minimum of a Bachelor’s degree if graduated before 1/1/2016.
Doctorate in Physical Therapy required if graduated after 1/1/2016.
International candidates must hold at least a Bachelor’s degree (Doctorate preferred).
Licensure/Certification:
Must hold or be eligible to obtain a Physical Therapist license in the state where services will be provided, with licensure required within the first 90 days of employment.
CPR certification is required within the first 90 days of employment.
Must maintain all certifications and licensure as they expire.
APTA membership preferred.
If driving a company vehicle, a valid driver’s license is required; if driving a personal vehicle, proof of insurance is also required.
Experience:
Work experience preferred but not required, beyond clinical training as part of the educational program.
Other Requirements:
Must be able to lift 50 pounds, assist with 100 pounds, and may be required to push up to 300 pounds.
Willingness to work at other locations as assigned.
