Position Summary:
Responsible for providing primary care in a family-based clinic environment within an interdisciplinary team to achieve HealthPoint mission and goals. Utilizing nursing standards of care including assessment and analysis, diagnosis/problem identification, planning, evaluation and implementation of patient care across the life span with an emphasis on chronic care management. in a clinic, virtual or community/shelter settings.
Compensation is dependent on skills and experience.
Flexible scheduling available! Clinic is open 7 days week. Schedule can be two 12's, and rotating weekends or four 10's.
Your contribution to the team includes:
Use client data and nursing scientific principles to develop nursing diagnosis and to identify client problems to deliver effective nursing care.Implement the plan of care by initiating nursing interventions through giving direct care and supervising other members of the care team.Triage of phone calls and walk-in patients for acuteness of need and/or priority of intervention and document assessment information of all activities.Perform complex procedures as appropriate for the RN scope of licensure and the individual RN's competency (may include wound care, IV insertion and fluid/medication administration, urinary catheter insertion and removal). Administer select injections and medications.Educate patients with a focus on medications, abnormal laboratory results, self-management of acute and chronic conditions.Promote wellness and the importance of preventive care across the life span.Provide motivational interviewing and harm reduction counselingProvide support and education of Medical Assistants that includes mentoring, training and oversight in basic skills and procedures, testing knowledge base and giving in-services.Participate in implementation of clinical programs for patients with chronic conditions.The float Nurse Clinician II will work throughout various HealthPoint locations to provide coverage for shortages as well as providing additional support with other assigned duties. Responsible for monitoring and staying current on daily schedule. Must be flexible and able to adapt to multiple clinic environments and patient populations while maintain a high level of professionalism and quality patient care.Maintain good attendance, is punctual and works full scheduled shift is a condition of employment.Demonstrates respectful, professional and appropriate behavior that supports a team-oriented work environment.Demonstrate a commitment to the mission, core values and goals of HealthPoint and its healthcare delivery including the ability to integrate values of justice, respect, compassion, excellence and stewardship into appropriate programs and services.Perform other duties as assigned by supervisor.SECONDARY DUTIES AND RESPONSIBILITIES
Participate in orientation of newly hired Nurse Clinicians.Provide OB/GYN patient centered care.Attend staff meetings, in-service meetings and participate in agency committees and task force activities as required.Must have's you'll need to be successful:
Minimum BSN five (5) years of experience or 15 years equivalent and advanced degree or certification required. Degree in Nursing from an accredited college or university.Unencumbered Washington State Registered Nurse license required. National Certification in Ambulatory Care or similar type certification desired.Ability to read and interpret technical and other complex documents. Ability to write routine correspondence such as letters and memos. Ability to present information in one-on-one and small group situations to internal and/or external clients.Ability to apply principals of logical or scientific thinking to a wide range of intellectual and practical problems. Work situations are vaguely defined, often unprecedented, and unique in character. Problems are very complex, abstract, conceptual, and long-term in nature. There is a continual requirement for strategic, innovative thought and synthesis of information as it pertains to work situations.Work situations require consideration and interpretation of circumstances or information to choose the most effective response. Solutions may be technical yet relatively straightforward and well-defined once problems are understood.Possesses basic operating knowledge of computers. Beginning level of Word, Excel and Outlook required. Experience with electronic medical records required.Proof of vaccination for COVID-19 is required, prior to start. HealthPoint does not accept the Johnson & Johnson COVID-19 vaccine as proof of vaccination. If you have received the Johnson & Johnson vaccine, we ask that you provide documentation demonstrating proof of an alternate COVID vaccine or vaccine series. All new employees are also required to show proof of immunizations and/or immunity to MMR (measles, mumps, rubella), Varicella, annual Influenza and TB QuantiFERON Gold Titer. Additionally, if you work in a HealthPoint clinic, Tdap (within last 10 years) is required. Hepatitis B. is required for clinical employees with potential exposure to blood/blood products. All immunizations are a condition of employment. Upon hire, employees must provide proof of their immunizations and/or immune titer results prior to starting or no later than their fifth (5) business day of employment.
