Job Description

PURPOSE OF POSITION

Under direction, with wide latitude for the exercise of independent initiative and judgment, supervises a Pharmacy unit, clinic or other health care setting. The Level 3 IV admixture and

Oncology supervisor will perform a wide variety of duties to help meet the pharmaceutical needs of the hospital and its patients focusing on supervising all areas of the IV room operation and its components.

IV and Oncology Services Supervisor

SUMMARY OF DUTIES AND RESPONSIBILITIES

In conjunction with the Associate Director, Oncology and IV Admixture Service Supervisor is responsible for coordinating operations and processes relating to these services.Serves as a liaison between pharmacy and hospital staff for integration of sterile compounding activities and regulatory compliance.Oncology and IV Admixture Service Supervisor is responsible for aseptic preparation of intravenous infusions and supportive care drugs; drug and supply ordering; repackaging; restocking; maintaining a lean and accurate inventory; removal of expired drugs; and acting as a liaison between clinical and non-clinical staff.Responsible for ensuring all compounding rooms, devices, supplies, and personnel are compliant with USP Chapter and USP Chapter regulations as well as any applicable state Board of Pharmacy regulationsCompounds, manipulates and dispenses sterile and non-sterile medication, including hazardous medication, according to USP 795, 797 and/or 800 guidelines.Responsible for the training and maintaining proper credentialing of personnel.Responsible for maintaining all documentation, written and electronic including QA activity, temperature logs, and activity logsReviews written/electronic medication orders or prescriptions to determine appropriate ingredients, strength, and dosage. Reviews medication orders for: potential allergies, drug/drug and food/drug interactions, route of administration, appropriate dose and potential for adverse drug reactions with other medications orderedResponsible for ensuring that all sterile and non-sterile compounded items behave appropriately in Epic.Contacts the prescriber, as appropriate, to clarify orders and/or make recommendations concerning alternate therapiesCommunicates with Clinical Pharmacist on any clinical issues regarding patient's ordersParticipates in the development of continuous quality improvement initiatives in the areas of IV and Oncolo ServicesProvides input on the development of policies and procedures related to sterile compounding and IV and Oncology Services.Monitors, evaluates and addresses staff performance and follows appropriate disciplinary proceedings, as necessary. Enforcement of time and attendance policies.Responsible for oversight of pharmacists' and pharmacy technicians, all sterile compounding/ distributive activities and management of the drug purchasing/inventoryPrepares unit work schedule and adjusts assignments to facilitate workflow and ensure that work is completed in a timely manner.Works to resolve problems independently and escalates matters that require the attention of the managers and other staff members.Ensures adequate communication and coordination between staff and management, units of operation, and tours through the use of huddles, email, schedules, policies and meetings.Responsible for extensive knowledge and the implementation of downtime procedures.Performs all other related duties as assignedMinimum Qualifications

For Level I:1. Possession of a valid NY State license to practice pharmacy.

For Level II: In addition to meeting the qualification requirements of the lower level:1. Two years of post licensure experience; or2. Completion of a post graduate specialty residency or fellowship program; or3. Current Pharmacy Board Certification.

For Level III: In addition to meeting the qualification requirements of the lower levels, an additional:1. One year of post licensure supervisory experience or specialty clinical experience.
