Duties:
Effectively screens/evaluates patients within short-term and long-term rehab and opportunity for outpatientUtilizes standardized assessments and evidence-based practice to support clinical interventionsProvides rehabilitative, skilled, and medically necessary treatment intervention to patients within short and long-term settingsAdministers a wide variety of interventions as part of the plan of care based upon individual patient needsAssesses patient's progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirementsProvides consultation and counseling to patients, as well as families, caregivers and other service providers related to the physical disordersEffectively communicates with the OneClinical Team regarding patient progress and barriersProvides comprehensive discharge summary of services provided per regulatory and clinical requirements
Qualifications:
Licensed Physical TherapistThis position is open to PTs at all experience levels.
