DEPARTMENTAL SUMMARY:
The Rehabilitation Department provides therapy and nursing services to patients, families and patient's caregivers including: Rehabilitation Nursing Care, Physical Therapy, Occupational Therapy and Speech Language Pathology. Our department operates as a system department and provides services to Acute Care, Inpatient Rehabilitation Facility, Home Health and Outpatient settings across four campuses. Our Rehabilitation departments offer a variety of specialty services across the various services delivery areas.
POSITION OVERVIEW: The Physical Therapist
evaluates, plans, and provides treatment and rehabilitation as prescribed by a referring physician. Works with family and team members regarding patient's abilities and goals.
This position does not directly manage any other caregivers.
ESSENTIAL FUNCTIONS AND DUTIES:
Adheres to Oregon State Physical Therapy Licensing Board.
Provides physical therapy evaluation and treatment to achieve desired outcomes.
Provides staff consultation and training.
Provides community resource knowledge and referral services.
Provides physical therapy services to other hospital service areas.
May be required to drive Rehabilitation van and meet all related SCHS vehicle policy requirements.
Functions as a member of the Interdisciplinary Care Team to assure patient care is accomplished effectively and efficiently in a cost effective manner.
Participates in clinical care conferencing.
Gathers data and accurately documents information in a timely manner demonstrating compliance with departmental and professional regulations.
Participates with team members in facilitating patient's and family's learning throughout continuum of care. Reinforces patient's continued health care through teaching and/or referral to community agency follow-up.
Supports the vision, mission and values of the organization in all respects.
Supports Lean principles of continuous improvement with energy and enthusiasm, functioning as a champion of change.
Provides and maintains a safe environment for caregivers, patients and guests.
Conducts all activities with the highest standards of professionalism and confidentiality. EDUCATION:
Required : Bachelors, Masters, or Doctorate degree in Physical Therapy from a college or university accredited by the American Physical Therapy Association.

LICENSURE/CERTIFICATION/REGISTRATION:
Required : Must have necessary requisites for licensure as a Physical Therapist by the Oregon State Board of Health. Valid Oregon driver's license and ability to meet St. Charles Health System driving requirements. Ability to travel to business functions/trainings/meetings and all St. Charles Health System worksites.
Current AHA Basic Life Support for Healthcare Provider certification
Preferred : Preferably a member in good standing of the American Physical Therapy Association
EXPERIENCE:
Required : Must have one year experience with therapy procedures, current orthopedic and neurological follow-up procedures, and rehabilitation practices.
Preferred : Previous physical therapy experience in hospital-based setting.
PERSONAL PROTECTIVE EQUIPMENT:
Must be able to wear appropriate Personal Protective Equipment (PPE) required to perform the job safely.
