Position Summary: We are seeking a Ful-Time dedicated and compassionate Home Hospice RN Case Manager to join our team in Green Valley, AZ and surrounding areas. The ideal candidate will have a strong background in hospice care and be committed to providing high-quality, patient-centered care.
**$5,000 bonus**
Key Responsibilities:
Provide direct patient care in the home setting, managing symptoms and addressing patient and family needs. Develop and implement individualized care plans based on patient assessments. Coordinate with interdisciplinary team members, including physicians, social workers, and chaplains, to ensure comprehensive care. Educate patients and families on disease progression, symptom management, and end-of-life care. Monitor and document patient progress, making adjustments to care plans as necessary. Participate in on-call rotation to provide support to patients and families as needed. Qualifications:
Active RN license in the state of Arizona. Minimum of 1 year of hospice nursing experience. Strong clinical assessment and critical thinking skills. Excellent communication and interpersonal skills. Ability to work independently and as part of a team. Compassionate and empathetic approach to patient care. Valid driver's license and reliable transportation. Benefits:
