Under the general direction of the Home Health Administrator, the Physical Therapist evaluates and treats patients recovering from injury, disease, or surgery in a home care environment. Establishes treatment plans and treats patients using a variety of methods, including exercises, stretching maneuvers, hands-on therapy, and equipment to ease patients' pain and help them increase their mobility. Works cooperatively with physicians, nurses, case managers, and adjustors. Documents and reports on patients' progress. Monitors, documents, and reports patients' progress.
Duties and Responsibilities:• Conduct an in-person comprehensive physical therapy assessment on Better Life patients, as assigned• Coordinate with the case management team to develop a comprehensive plan of care for each patient• Conduct an in-person physical therapy assessment or reassessment, as needed• Participate as a member of a case management team, communicating patient changes, collaborating on care planning decisions and coordination of 24-hour care delivery• Actively participate in the Intake and Assessment meetings• Evaluate patients to determine and assure appropriate prescription, needs, goals, history, complications, and precautions• Follow-up with patient during treatment, explain precautions to take• Discusses condition and treatment plans and recommends solutions to patients care problems with the case management team• Assess the need for adaptive durable medical equipment, specialized shoes, and prosthesis• Assess patient's home for safety and mobility• Recommend home modifications and or equipment• Plan physical therapy treatment programs according to recognized standards and policies• Administer physical therapy treatment programs consistent in achieving identified goals and according to recognized standards and policies• Confer with case management team on patient care issues• Document evaluation, assessment, plan, and treatment rendered accurately and in timely according to Better Life Home Health policy• Measure effectiveness of treatment, document, and revise plan accordingly• Educate and instruct patients/family in procedures, home programs, and document training and understanding by patient/family• Confers with physicians and other disciplines to assure complete and integrated program• Completes necessary documentation and statistics accurately and in a timely fashion• Direct, supervise, and perform initial and annual competencies, and evaluates the performance of the licensed Physical Therapist Assistant (PTA)• Review and cosign entries of the PTA into the patient medical record• Review the plan of care with the PTA before the PTA provides care• At least once every 30-days after new or revised plans of care, document in the medical record that the plan and the patient's progress have been reviewed with the PTA• Train Better Life Home Health staff in body mechanics, use of equipment, transfers, gait patterns, and methods to maximize safety and functional abilities in performing activities of daily living (ADL's)• Perform initial and annual competencies on direct care staff under supervision• Participate in and support Quality Initiatives• Contribute to research and other department activities as assigned• Provide age specific competent care to all persons served• Know and adhere to philosophy and goals of Better Life Home Health• Initiate/suggest process improvement and/or cost saving measures that enhance patient care• Demonstrate clinical excellence and utilize insights in timely response to patient's needs• Proactively involve the healthcare team in plan of care• Assist in the implementation of department competency plan• Develop patient educational material• Maintain professional level and remains current with new advances• Adhere to professional, legal and safety standards Act only within the scope of his or her authority to practice• Collaborate with other members of the in-home care team within your assigned territory• Back-up fellow healthcare representatives as needed for special events or vacation time• Maintain memberships in related health care professional organizations for the purpose of networking• Communicate issues that arise in the field with clinical staff through defined channels to enhance service delivery• Participate in organizational committees as identified that enhance the working relationships between departments within Better Life Home Health• At all times, represent Better Life Home Health professionally through care and/or services provided to all clients and patients• Comply with Better Life Home Health policies, procedures, and standard practices• Maintain the confidentiality of patients and families within Better Life Home Health as per government mandates• Use resources in a fiscally responsible manner• Promote Better Life Home Health through participation in community and professional organizations• Improve own professional knowledge and skill level• Additional duties, as assigned
Qualifications and Requirements:
BSPT, MSPT or DPT degree requiredCurrent licensure as a Physical Therapist in the state of FloridaMinimum two years' experience in a Home Health or Home Care settingCurrent State of Florida driver's license with driving record acceptable to Alivia Care insurance carrier, and reliable transportationCurrent Auto Insurance coverageCurrent CPR certificationLevel II AHCA (Agency for Healthcare Administration Background ScreeningBi-lingual Spanish a plusSkilled at:
