Doctor of Medicine (MD) degree or Doctor of Osteopathic Medicine (DO) degree from an accredited medical school.Residency training program in anesthesiology accredited by the Accreditation Council for Graduate Medical Education (ACGME) or the AOA.License & Certifications

Board certification in anesthesiology by the American Board of Anesthesiology (ABA) or the American Osteopathic Association (AOA).Active and unrestricted medical license in Indiana .Experience
