Description
Licensed Clinical Social Worker provides directly or assists program staff in completion of the health and behavior screening, counseling and referral. If indicated, provides further psycho-social assessment, including diagnosis, treatment plan, and individual therapy to patients in outpatient setting.
Provides education or assists staff in developing education sessions on stress and anger management. Determines tools to be used for screening /assessing anxiety and depression, determines criteria to be used to assist staff in referring patients for further health and behavior services.
Psychosocial assessments and diagnostic evaluations with referred patients to develop a treatment plan. Provides individual counseling and/or educational services to patients referred. Coordinates and communicates activities related to patient plan of care/services rendered with the interdisciplinary team.
Maintains clinical and professional competency according to licensure requirements for Licensed Clinical Social Workers in the state of North Carolina and provides care appropriate for the age, health, and developmental status of patients served. Has full access to patient health information.
Requirements
Masters in Social Work, Psychology or Behavioral Health from an accredited program. LCSW and board certification required.Broad knowledge of health and behavior therapy, crisis intervention, and psychosocial assessment and diagnosis.Excellent communication, organizational, problem solving, and decision-making skills. Functions independently and in teams. Has the ability to interpret information to identify each patient's requirements for interventions. Ability to successfully complete generic and department-specific skills validation and competency testing.Good physical and mental fitness to meet the demands of emergency situations. Emotional maturity and stability.Must possess full range of body motion to pass basic FIT test for position to include walking, kneeling, standing, pushing, pulling, bending, stooping, reaching and sitting for extended periods of time. Must be able to lift and carry up to 30 pounds.
