The
Program of All-Inclusive Care for the Elderly (P.A.C.E.)
is seeking a licensed Physical Therapist to join our interdisciplinary care team. P.A.C.E. is an alternative to nursing home care, enabling elderly individuals to live independently in the community while receiving comprehensive health services. As a Physical Therapist within this program, you will work closely with a team of physicians, pharmacists, nurses, social workers, therapists, and aides to provide tailored care to the elderly.
Key Responsibilities:
Provide Comprehensive Physical Therapy Care:
Deliver physical therapy services to elderly participants in coordination with the interdisciplinary care team (IDT).Assess, plan, and implement individualized therapy treatment programs that help participants maintain or improve their functional independence.
Collaborate with Care Team:
Work closely with the IDT, including physicians, nurses, and social workers, to address participants' specific health and rehabilitation needs.
Patient Education and Support:
Educate participants and caregivers on proper exercises, mobility techniques, and home care plans to promote continued independence and safety at home.
Support P.A.C.E. Mission:
Contribute to P.A.C.E.'s mission of helping elderly individuals live independently in their communities, avoiding the need for nursing home care whenever possible.
