Job Core Summary
The Social Worker, LCSW works with clients in order to help them deal with issues involving mental and emotional health. This position provides emotional support, mental health evaluations and therapy, and case management services.
Core Responsibilities
Advocates for and helps clients gain access to resources while confronting their personal issues.
Counsels and provides crisis intervention for patients, including assessment and treatment of behavioral problems.
Coaches patients on how to manage ongoing health conditions. Provides therapy to help patients make positive behavior changes.
Assists patients and their families through individual, group, and education conferences to understand, accept, and follow treatment recommendations.
Department Specific Functions
Social Worker provides patients, families, and caregivers with the psychosocial support needed to cope with chronic, acute, or terminal illnesses. Services include advising family caregivers, providing patient education and counseling, and making necessary referrals for other social services . The Social Worker works with members of the multidisciplinary team to further understand significant social and emotional factors underlying a patient's health and/or attitude problems. Work is performed according to established policies and procedures and in accordance with professional practice.
Responsible for social work services at the multidisciplinary ALS and MDA multispecialty and regular clinics.
Conducts suitability assessments of patients in the multidisciplinary clinic.
Provides psycho-social assessment and diagnosis of behavioral disabilities with recommendations.
Provides information and referral to public or private agencies with appropriate follow-up.
Provides information to patients about appropriate insurances and disability process.
Responsible for completing disability forms, workers compensation, Medicaid waiver forms and other governmental program forms.
Provides information and referrals of other agencies that may offer services for patients with ALS.
Provide brief support and counseling to patients and families to help adjust and adapt to a changed lifestyle.
Identify patient abuse or neglect and make contact with state investigating agency when appropriate- reports abuse as identified.
Identify environmental impediments to patient progress through interview and review of patient records
Assist with special projects and other duties as assigned.
Maintains confidentiality of all patients’ information according to HIPPA guidelines from University of Miami.

Core Qualifications
Master’s Degree in Social Work
Certification in relevant specialty or field. Encouraged to still apply if applicant does not have certificate or license.
Minimum 1 year of experience
Bilingual (English/Spanish) highly preferred

Ability to communicate effectively in both oral and written form.
Ability to handle difficult and stressful situations with professional composure.
Ability to maintain effective interpersonal relationships.
