Job Description:

Job Title: Hospice RN Case Manager - Van Buren County

Schedule: Monday through Friday, from 8:00a to 5:00p

Coverage Area: 94 Corridor > New Buffalo up to Benton Harbor

Key Responsibilities:

As a RN Case Manager you will:

Assess and evaluate the physical, psychosocial, and environmental needs of your assigned home hospice patients, using clinical records, on-site visits, and team consultations.

Conduct initial and ongoing evaluations of each patient’s nursing needs during every visit.

Communicate significant patient updates or changes in health to the appropriate clinical supervisor, physician, or team members involved in the patient’s care.

Implement safety and care plans, offering guidance and instruction to patients and their families based on specific care needs.

Ensure quality care is delivered by participating in the execution of individualized plans of care, focused on achieving positive patient outcomes.

Recognize and respond to any changes in a patient’s condition, initiating appropriate actions when necessary.

Confirm and follow the patient’s Plan of Care prior to every visit, providing care according to the physician's orders, clinical assessments, and best practices.

Take on additional responsibilities and projects as assigned.

What you'll Need to Bring:

Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing

Current State Licensee as a Registered Nurse RN

1 year of experience in a clinical care setting

Experience in hospice or home health environment is preferred

Nursing skills as defined by accepted nursing standards

Ability to handle multiple priorities, documentation requirements, and deadlines

Reliable transportation to perform job tasks
