Position Summary:
Works with the Home Care Director and Home Care Manager to ensure effective coordination and delivery of PACE home care services including Home Nursing, ADL and IADL support services including home monitoring, health aides, homemakers, laundry, meals and other services as required. In coordination with the PACE Interdisciplinary Teams (IDT), providing assistance with oversight of health aide and nurse scheduling, and providing direct supervision of health aides. Works on performance improvement and other initiatives that may be identified through program, performance, financial and other monitoring.
Essential Duties & Responsibilities:Provides direct supervision to an identified group of Health aides, providing oversight in all aspects of their job performance.Along with the Home Care Manager/ Residential Nurse Manager , investigates grievances and incidents in accordance with ESP Policy and Procedure, addressing individual performance and systems changes as indicated by the results of the investigation and resolution. Makes reports through QAPI or other avenues as required.Participants in the annual competency review and training program, ensuring that the training and performance review tools are updated at least annually to include new or changed policies, procedures, skills requirements or equipment.Assist the Home Care Manager/ Residential Nurse Manager in conducting periodic audits to 1) ensure that Personal Service Plans are updated and consistent with current clinical and functional status; and 2) health aides are providing care to participants consistent with service plans 3.) Other audits as assigned.Provides timely performance evaluations and disciplinary actions for direct reports. Ensures that direct reports meet competency, infection control, and safety requirements (CPR, drills, etc).Supervises health aides in accordance with ESP Policy and Procedure. Conducts supervisory visits with health aides on a routine basis and as needed to address specific concerns or to oversee implementation of new procedures, skills, or use of new equipment.Works with Home Care staff to regularly monitor health aide schedules and assignments to ensure efficient and effective care for ESP participants.Participates in long range planning and determining staffing needs.Assists Home Care Manager/Residential Nurse manager to monitor utilization and processes for stocking Nursing and health aide supplies. Makes recommendations for improvement as required to maintain appropriate levels of supplies.Works closely with interdisciplinary team to insure good communicationServes as a resource to the teams for health aide issues and concerns.Participates in regularly scheduled and as needed health aide meetings.Meets regularly with Managers and keeps Managers informed and involved when appropriate.Serves as a resource for both staff and participant issues.Provides initial supervision of contract aides at Winthrop Place and in the community as indicated.Participates in the orientation of new aides including but not limited to ESP Policies and Procedures and Homecare Department workflows. Oversees site-specific orientation to physical environment, introductions to participants, and orientation to individual care plans.Participates in the oversight of performance and monitors quality of service.Assumes the role of Home Care Coordinator as assigned by Home Care Director including: Conducts in-home functional, medication management, and home safety evaluations as part of intake or initial, status change, scheduled (biannual) assessment, or as required; As part of IDT, participates in discussions of comprehensive assessment, identification of active problems, requirements for stable monitoring; makes recommendations for in-home services, ADL assistance and follow up in day center, and other interventions as may be required to address functional deficits and other nursing needs. Monitors implementation of home care interventions; Identifies changes in home environment, ADL/IADL requirements and functional and home safety issues; adjusts care plan or reports concerns to IDT as required.Along with the Home Care Manager/ Residential Nurse Manager, monitors quality of care and performance improvement initiatives as needed to address concerns, deficiencies, or trends.Oversees record documentation for completeness and quality.Identifies and implements home health aide specific QI projects.Assists in policy review and revision. Works with Home Care Director/Residential Nurse Manager to update policies, procedures and workflows as needed to ensure consistency with practice, changes in regulatory or other requirements.Reviews grievance, incident, performance and other trends to identify opportunities for improvement. Makes recommendations for addressing systems issues through QAPI or ESP Administrative processes.Attends QAPI and Leadership meetings as required.Involved in participant/family satisfaction evaluations.Qualifications and Requirements:
BS in Nursing or equivalent experience or certification.Current MA Nursing License.Strong clinical background in geriatric care.Home health care management preferred.Expertise dealing with elders and families.Benefits:

Starting at $43.77 up to $70.03 depending on experience
