As a part of Uptown Community Health Center, Caritas Internal Medicine sees patients of 18 years and older from all backgrounds of life for Primary Care services. As the main clinic for a Residency program we have over 30 different Providers who rotate through our clinic.

Under the direction of the Chief Medical Officer, this position provides comprehensive, coordinated primary and preventive healthcare services to an ethnically diverse patient population within their appropriate scope of practice.

Oversee Resident Physicians

Evaluates and treats patients within the licensed scope of practice; appropriately refers patients for specialty evaluation and treatment.

Provides preventive health advice and case management and provides consultation to other staff.

Provides or arranges for comprehensive primary care services in a primary medical care ambulatory clinic setting, including laboratory and diagnostic workup and specialty referrals.

Participates in multi-disciplinary, team-based care of patients.

Responsible for accurate, timely, and complete documentation in patient records in accordance with Uptown Community Health Center policy.

Renders care to UCHC patients in accordance with quality and standards as established by UCHC, the standards of care prevailing in the community, applicable law, and according to state and federal grant requirements for non-discrimination regardless of status, race, sex, religion, national origin, handicap, age, or ability to pay.

Provide patients and family members with counseling and education regarding health maintenance, disease prevention, diagnosis, treatment, and the need for follow-up as appropriate during each patient visit. Verify and document that the patient understands the diagnosis, treatment, and follow-up recommendations.

Serves as a role model for effective staff teamwork and actively maintains positive morale and attitude.

Licensure:

Valid license to practice the profession in the applicable state.

Board certification/eligibility based on education and specialty.

Valid DEA in the applicable state

Qualifications:

Required:

MD or DO degree from a fully accredited medical or osteopathic school of medicine.

Residency training in the area of specialtyPreferred:

Experience with Epic application and health record.

Experience working with and sensitivity toward special populations, including those with limited income, limited education, and non-English speaking individuals.

Bilingual English / Spanish, complete comprehension, literacy, and fluency in both languages.
