Job Description :We are seeking a dedicated and skilled
Physical Therapist Assistant (PTA)
for a position at a
Skilled Nursing Facility (SNF)

Walnut Creek, CA . In this role, you will provide essential therapy services to help residents regain strength, improve mobility, and enhance their quality of life. You will collaborate closely with licensed physical therapists to implement individualized treatment plans and monitor patient progress.
Key Responsibilities :Assist in implementing treatment plans developed by supervising physical therapists, tailored to each resident's needs.Provide direct therapy services within the skilled nursing facility, including therapeutic exercises, functional training, and mobility assistance.Monitor and document patient progress accurately, adjusting treatment approaches as necessary under the direction of the physical therapist.Educate residents and their families on exercises and techniques to facilitate continued therapy beyond scheduled visits.Work collaboratively with the interdisciplinary team to ensure comprehensive and coordinated care for all residents.Maintain timely and accurate records in compliance with organizational and regulatory standards.Uphold all professional, ethical, and legal standards in the delivery of care within the SNF environment.Qualifications :
Minimum of
1 year of experience
as a Physical Therapist Assistant (PTA) - Required.Active
California PTA license
