Services are individualized according to the strengths and needs of each child, youth or young adult with a goal of supporting and sustaining them in their current family system, or in the least restrictive and most clinically appropriate environment for their well-being.This Case Manager will help link Clients to resources promoting a total wellness-focused lifestyle within a holistic perspective; they will work under a truly dynamic TEAM approach and play a first-hand role in enhancing the lives of local families.POSITION DETAILS:Improving clients' functional abilities who have complex needs and providing appropriate case management services.Coordinating treatment and supportive services to promote optimal community integration.Providing referral, advocacy and case coordination services in a variety of settings.Linking responsibilities; accessing/utilizing other services, contacting DCAF agencies, job coaching duties, vocational readiness training, housing, AFDC, Food Stamps, etc.Developing natural support systems and fostering both transitional and independent living skills.Service planning and monthly home visits promoting family participation.Thriving as part of a multi-disciplinary Children's Community Services Team.Other duties as assigned by the Supervisor of Children's Community Services.REQUIREMENTS:Bachelor's Degree in Clinical Psychology, Social Work, Behavioral Science, Rehabilitation Counseling or other relevant field = RequiredCurrent/Clear/Active FL Driver's License = RequiredSpanish Bilingual = RequiredAt least a year of related or relevant experience = RequiredAbility to function at a high level in a demanding community environment = RequiredKnowledge of community services and resources = PreferredA "How can I help?" attitude when it comes to working across programs = RequiredAbility to relate to a wide variety of co-workers, clients, and community agencies = RequiredAccurate, organized and informative record-keeping/reporting skills = RequiredAbility to ensure that all activities are managed according to David Lawrence Center policy and procedure = RequiredMicrosoft Office 365 skills (Including Teams/Video Conferencing) and Electronic Medical Records experience = Strongly PreferredStaying compliant on all trainings and immunizations = RequiredAbility to pass a Level II DCF Mental Health Fingerprinting, Drug Screen and Local Criminal Check = RequiredLOGISTICS & COMPENSATION:- Full-Time Hourly/Non-Exempt Position~ Monday-Friday, Target Standard Business Hours~ Schedule flexibility required for Client and Team needs~ Local travel required in Collier County- COMPREHENSIVE benefits outside of competitive salary include:~ Exciting 401(k) w/Employer Match~ Employer Paid Life/AD&D~ Generous PTO & Paid Holidays~ Medical/Dental/Vision Insurance~ Diverse discounts with local businesses~ DLC-sponsored Health & Wellness Initiatives Including EAP~ Extensive Tuition/Licensure Reimbursement Plans/Loan ForgivenessADDITIONAL REWARDS:DLC is a symbol of hope for children, adolescents, adults and the elderly residing in Southwest Florida who struggle with mental health and substance use challenges.Through DLC's work, lives are saved, families are kept together, children succeed, homelessness is reduced, quality of life is returned, and dignity is restored to those in need.Together we can inspire and create an environment that empowers people towards life-changing wellness to achieve what is possible!If you are interested in making a commitment to providing exceptionally compassionate care and making a powerful difference in the lives of our clients and their families, you may contact HR Recruiter Angie Wester at
