Rate Caps for Submission:Malpractice - Must be included in rates.Orientation - half the hourly/callback rate*** kicks in after 8 hours. "Per hour of orientation or training beyond 8 hours"Hourly - ***-240 - "Per hour, not OT"Holiday - Must be 1.5x the regular rates "Per holiday hour, no OT"

Schedule/Availability Requirements:ASAP- as soon as credentialed - 6 months to a year• Shift times: Weekdays: 8:00 am to 5:00 pm, with a 1 hour lunch• Frequency: Monday - Friday, every week- no call, would also consider someone who could do only a few days/weekFacility Location: 1595 Soquel, Suite 400, Santa Cruz, CASetting: Internal Medicine- will consider a strong FP but that is not ideal.FTE: 1.0# Providers Needed: 1Duties: Typical IM clinic population average age 70. About half male/female with multiple comorbidities, such as diabetes, arthritis, heart disease, chf, etcRequired Procedures: N/AEMR: CernerPatients per Day: Other providers seeing 12-18 a day depending upon complexity (includes new patients)Support Staff: MA's, MOR's, APP (one in clinic) and RN assigned to clinicReason for Coverage: Provider RetiringDeal Breakers? Travel Preferences (see attachment for specific travel guidelines):• Preferred Lodging: none.
