Job Title: Physical Therapist - Home Health Job Specialty: Home Health PT Job Duration: 13 weeks Shift: Day 5x8-Hour (08:00 - 16:30) Guaranteed Hours: Not specified Experience: Minimum 2 years experience required License: Valid NH state license Certifications: AHA and Red Cross certs accepted Must-Have: - Epic, Home Health, Body Mechanics, Skin Integrity, Strength Testing, Vital Signs, Assessment, Discharge, Evaluation, Long Term Goals, OASIS, Plan of Care, Reassessments, Short Term Goals, Time Management, Home Exercise Program, Medications, Patient Family Education, Signs & Symptoms of physical, sexual, psychological abuse, Safety and Injury Prevention Job Description: - Conduct lifting and transfer of patients, patient transport to/from treatment- Schedule appointments and manage telephone calls- Occasional weekends as needed- General skills required for home health duties- Preferred skills include experience in Cardiopulmonary, Geriatrics, Neurology, Oncology, Orthopedics, Pediatrics, Sports Medicine, Women's Health, and Wound Care- Utilize adaptive devices and conduct AROM/AAROM/PROM- Assess pain and sensory response- Identify environmental barriers and assist with assistive devices- Understand RUG levels and work with various types of insurance, including Medicare A, Medicare B, Medicaid, Commercial, Private Pay, and Worker's Compensation- Experience with lifting and transferring patients, patient transport, scheduling, and conducting telephone calls- Support may be required for additional units such as ER, ICU, and Med/Surg- Use of Epic for medication, documentation, and communication systems- COVID-19 Vaccination: Required (medical/religious exemptions only)Please note that former employees of associated facilities are not considered for this role.
