The mission of Lyon-Martin Community Health Services is to provide high quality, compassionate and trauma-informed medical, gynecological, and mental health care services to trans, non-binary, gender non-conforming, and intersex communities and cis-gender women with specific sensitivity to LGBQ sexual orientation, disability, size, race, ethnicity, and language, regardless of immigration status or ability to pay.

We aim to address and eliminate health inequities for our communities by rooting our clinical practice in core foundational intersectional frameworks of anti-racism, self-determination, harm reduction, and reproductive and disability justice. Job Summary

The Director of Nursing acts as a clinical leader by overseeing of all aspects of clinical care within the nursing scope of practice throughout the clinic. The Director of Nursing is a key member of the Leadership Team, providing strategic vision, directly supervising the nursing team, including the Outreach RN, the electrologist, and the Medical Assistant Manager, providing coverage for nursing support to LMCHS patients using a trauma-informed, harm reduction focused Patient Centered Medical/Health Home delivery model. The Director of Nursing takes a leadership role in training programs including Lyon-Martin's student rotation, phlebotomy externship, and in supporting the on-boarding new clinical staff. They co-lead all Quality Improvement activities and Research programs in collaboration with the Medical Directors, Director of Programs, Director of Operations, and QI Manager, and assist with the development and maintenance of policies and procedures. The Director of Nursing provides complex care management for patients with multiple chronic medical illnesses and behavioral health concerns to facilitate a collaborative, holistic and integrated model of care. The Director of Nursing is expected to uphold and imbue in others a commitment to addressing racism, ableism, fatphobia, gender and sexual discrimination within the organization and our clinical practice.

Key Responsibilities

Supervisor Responsibilities:

Develops and implements Policies & Procedures and forms pertaining to the Nursing and Medical Departments in collaboration with Leadership Team.Provides clinical leadership and oversight to nurses, electrologist, and students.Provides supervision of all nurses, electrologist, and MA manager.Promotes and enhances integrated team-based care in the provision of medical care, including patient centered care using trauma-informed care and harm reduction frameworks.In collaboration with Leadership Team, creates plans when an RNs and Providers are absent so that a coverage is available to review lab and diagnostic reports, prescriptions, and follow up on medical care for that panel.Thorough understanding of, oversight of, and promotion of continuous quality improvement efforts.Provides oversight for nursing peer review process.In collaboration with Leadership Team, hires, orients, trains, schedules, and terminates nurses and electrologist(s).Monitors performance through chart audits, annual performance appraisal of all direct reports. Provides feedback and makes recommendations for individual performance improvement and professional development.Increases staff competencies and satisfaction through training and mentorship and improves the overall quality and efficiency of clinical workflows.Clinical Care Responsibilities:

Provide coverage of nursing care as part of an integrated care team, within the scope of license and agency credentials, to patients within a Patient Centered Medical/Health Home delivery model, as needed.Demonstrates clinical expertise in primary healthcare for queer, transgender and intersex populations and gynecological care.Provides care, within the scope of credentialed privileges and standardized procedures, to patients within an integrated delivery model according to pre-arranged schedules, documenting care promptly and following charting protocols and applicable CA state licensing requirements; completes patient sessions in a timely manner; closes visit notes in a timely manner (within 24 hours)Informs clients of normal and abnormal lab results and follow up plans, as appropriate for scope or under guidance of a provider;Triages and responds to patient and colleague requests for information and assistance (e.g., form completion, prescription refills, etc.) in a timely manner;Works collaboratively with mental health staff to facilitate integration between primary care and mental health in order to improve patient outcomes;Obtains and maintains licensure and BCLS/CPR certification;Attends continuing professional education programs, at the request or with the approval of supervisor, demonstrating participation in required hours of continuing medical education according to California lawUtilizes motivational interviewing skills and encourage patient activation and self-management;Appropriately utilizes standing orders;Facilitates team functioning and efficiencyAssesses and provides nursing advice to patients who walk-in with medical complaints or who call-in with medical concerns seeking advice;As a part of the Integrated HealthCare Team, prioritizes and takes responsibility for patients with complex medical issues and follow up with all patients regarding recent emergency room visits and inpatient stays;Works in collaboration with Leadership Team to review and makes recommendations for changes to all forms and handouts stocked to ensure evidence-based, trans inclusive and patient-centered language is used.In collaboration with Leadership Team, creates and provides trainings and orientation to RNs and re: LMCHS specific policies, procedures and protocols to newly on-boarded clinical staff.Manages hair removal program operations.Quality Improvement:

Participates in Quality Improvement activities; act as change champion and lead of QI activities in collaboration with Quality Committee;Work closely with staff collecting and reporting data as necessary;Supports team in meeting benchmarks for applicable quality indicators as noted in the annual quality management plan including the maintenance of productivity standards, cycle times, no-show rates, and landing times;Along with colleagues, assists in continuous quality improvement through on-going problem/needs identification and problem solving and, as requested, acts as change champion or lead in clinical improvement activities, including PIP measures.Reviews and makes recommendations for changes to QI in-reach protocols to ensure trans inclusivity and sensitivity is maintained throughout all QI efforts.Research Program:

Attends Research Program meetings.Develop survey(s) to be disseminated to staff and community to drive the development of research questions.Based on survey results, develop research questions, methods.Executes research initiatives with team members.Report findings to clinic and spearheads any solution-based intervention efforts as needed.Collaborates with team for publishing as needed.Clinical Rotation & Externship Programs:

Review applications and make recommendations regarding acceptance decisions in collaboration with team.Create training and precepting schedules for those accepted as needed.On-board, train, and support students/residents engaged in the clinical rotation.Review training materials and make recommendations for changes to ensure they are up-to-date with best practices.Through pre-and post-rotation testing and student satisfaction survey, collect and monitor data around student/resident learning and satisfaction to ensure efficacy of the program.Organizational Responsibilities:
