The role you’ll contribute:
The Cardiac Rehab Nurse provides direct patient care and guidance to patients and families with cardiovascular disease.
He/she performs an assessment designed to gather data relevant to the patient’s individual needs and completes the cardiac rehab individualized treatment plan.
This includes assessment and evaluation of cognitive, social, functional, and psychological status.
The Cardiac Rehab Nurse performs cardiac monitoring of patients, instructs patients on proper use of the exercise equipment, adjusts exercise prescriptions as indicated, and assesses the patient’s response to exercise. They will provide cardiovascular education and support of lifestyle modifications to patients and their families.

Performs an admission assessment to gather information and complete the comprehensive cardiac rehab individualized treatment plan; performs on-going assessments and
progress evaluations accordingly
Ability to educate patients on cardiac medications in their use, administration, and side-effects
Conducts daily code cart and quality controls checklist appropriately
Provides information to potential patients at the bedside to encourage their participation in the phase II cardiac rehab program
Demonstrates excellent verbal phone skills to conduct follow up phone calls, insurance verification, and patient scheduling
Assesses the patient on an ongoing basis including physical symptomatology, behavioral life style habits, psychosocial needs, lab work and test results.
Notifies patient’s physicians as needed with any concerns or major changes in patient status
Assists in the development and implementation of the exercise prescription, assesses patient’s physiological response to exercise, and makes necessary adjustments
Monitor/interpret EKG/heart rhythms
Provides emergent care per standing orders
Demonstrates and instructs in the safe, and correct use of the exercise equipment
Provides and maintains equipment and supplies necessary for delivery of service
Demonstrates proper body mechanics in stretching and use of weights
Identifies educational needs and provides teaching necessary for each patient; continually provides support to patients as they practice new lifestyle modifications
Teaches formally in classes and informally one-on-one
Communicates effectively amongst cardiac rehab staff and referring providers
Demonstrates proficiency in regards to the computer documentation of the cardiac rehab exercise sessions
Accurately preforms charge entry
Willingness to identify problems and participate in problem solving process
Possesses excellent human relation skills as demonstrated by the ability to interface positively with all customers and employees
Must have the ability for work with frequent interruptions, under stress, and with minimal supervision
