Conduct comprehensive evaluations and develop individualized treatment plans.Deliver effective therapeutic exercises tailored to patient needs.Track and record patient progress, adjusting treatments as necessary.Educate patients and families on rehabilitation practices and equipment.Collaborate with multidisciplinary teams to enhance patient outcomes.Maintain accurate records in compliance with healthcare regulations.RequirementsValid Physical Therapist license in the state of practice.Bachelor's or Doctorate in Physical Therapy from an accredited institution.Experience in Home Health setting required.HomeCare HomeBase charting experience required.Exceptional communication and interpersonal skills.Ability to work autonomously and manage a diverse caseload.BLS certification is required.BenefitsWeekly Pay$2200-$2400/wkPatient Related Travel Mileage Reimbursement
