JOB SUMMARYThe Occupational Therapist is responsible for providing occupational therapy evaluation and treatments to home care/hospice patients. Responsible for timely and accurate communication and documentation. Promotes and develops occupational therapy services in home care/hospice.

One (1) year previous experience.Licensure/Certification/Registration:
Minnesota OT license, Wisconsin OT license within six (6) months of hire. Wisconsin Background Study upon hire and renewed per Wisconsin state law.
