Job Summary:
We are seeking a dedicated and compassionate Supervising Physical Therapist to join our team at Emerald Care Center Southwest, a leading skilled nursing facility committed to providing exceptional care to our residents. As a Physical Therapist, you will play a crucial role in assessing, planning, and implementing personalized rehabilitation programs designed to enhance the mobility, strength, and overall quality of life for our residents.

This position operates in a clinical healthcare environment.
Responsibilities:Assessment and Evaluation: Conduct comprehensive evaluations of residents to assess their physical abilities, limitations, and functional needs. Develop individualized treatment plans based on assessment findings and residents' goals.Treatment Planning and Implementation: Design and implement therapeutic exercise programs to improve strength, mobility, balance, and coordination. Utilize a variety of therapeutic techniques and modalities to address specific conditions and promote recovery. Educate residents and their families about exercises, techniques, and strategies for improving mobility and managing conditions.Documentation and Reporting: Maintain accurate and detailed documentation of assessments, treatment plans, progress notes, and discharge summaries. Monitor and report on residents' progress, adjusting treatment plans as needed to ensure optimal outcomes.Collaboration and Communication: Work closely with interdisciplinary teams, including physicians, nurses, occupational therapists, and other healthcare professionals to coordinate care and ensure a holistic approach to treatment. Communicate effectively with residents and their families to provide education, support, and updates on treatment progress.Compliance and Safety: Adhere to facility policies, procedures, and regulatory requirements related to physical therapy practices. Ensure a safe and supportive environment for residents during therapy sessions, adhering to infection control and safety protocols.Professional Development: Stay current with advancements in physical therapy techniques, best practices, and relevant research. Participate in continuing education opportunities and professional development activities to enhance skills and knowledge.Qualifications:
Education: Doctoral or Master's degree in Physical Therapy from an accredited program.Licensure: Current state licensure as a Physical Therapist.Experience: Previous experience in a skilled nursing facility or similar healthcare setting is preferred but not required.Skills:
Strong clinical assessment skillsExcellent interpersonal and communication skillsThe ability to work effectively as part of a team.Physical Requirements:
