We are seeking to add a positive and compassionate therapist as we work together to provide the best quality care and experience for our residents. This position is responsible to evaluate and provide PT treatments to residents using the most current techniques per physician order and established Plan of Care. Daily treatment notes and billing is required, as well as weekly clinical progress notes, and rehab discharge programs for each resident via Optima. Position requires PT Texas licensure, dependability, and discretion with regard for confidentiality and proficient computer skills.
Job Summary:
This position as a Physical Therapist is to screen, evaluate, provide treatment, supervise the administration of physical therapy treatment, and participate in discharge planning of residents where services are being provided. Therapy services shall be carried out in accordance with state and APTA guidelines as well as Morningside's policies and procedures.
Essential Job Functions:
Administer physical therapy screening, evaluation, and treatment to residents according to your scope of practice where therapy services are being provided as follows:
Determine and recommend physical therapy services in accordance with resident care plan per responsible physician.Obtain informed consent from the resident and/or POA as applicable.Perform initial and ongoing physical therapy evaluations on new admissions, referred residents, and re-evaluations as indicated. Evaluations should summarize the nature of the resident's needs specifying the physical therapy plan of care. Plan of care should include appropriate medical and treatment diagnosis, short and long-term goals, prognosis, results of evaluation, resident goals and discharge planning.Complete accurate and appropriate daily billing in Optima HCS or applicable system for residents on caseload.Document resident progress in daily treatment encounter notes, weekly progress reports, recertification documentation, and discharge summaries in Optima HCS or applicable system for each resident in which you provide therapy services.Provide ongoing treatment to residents using the most current techniques following appropriate physician orders, APTA guidelines, and the state practice act's code of conduct/standards.Follow established procedures for use of physical agent modalities.Participate in discharge planning of residents on caseload and provide rehab discharge programs as applicable.Participate in facility care plan meetings, weekly rehab team meeting, company webinars, and interdisciplinary team trainings/conferences.Adhere to HIPAA regulations.Adhere to the weekly productivity standard of a minimum of 85%.Adhere to other delegated tasks on the Evaluating Therapist Task List.
Supervise Physical Therapy Assistant (s)Supervise the PTA(s) based upon the State Practice ActMaintain Licensure and Company Requirements:
Obtain CEUs per State Board requirements annually/bi-annually.Renew licensure/certification prior to expiration.Ensure ongoing BLS certification is completed prior to expiration date.Ensure annual PPD requirements are completed prior to expiration date.Participate in program development, in-service training and educational programs related to Physical Therapy as applicable.
Adhere to Company Standards, Policies and Procedures:
Adhere to all company policies and procedures.Establish and maintain a good rapport with residents, physicians, co-workers, and facility staff.Communicate with Therapy Manager, co-workers, and facility team effectively and efficiently.Complete initial training and annual re-training on Dementia, Elder Abuse, Infection Control, Fire Safety, Emergency Preparedness, HIPAA, OSHA, Right to Know Law, etc.Participate in periodic clinical skills review of Physical Therapy Staff throughout the year in conjunction with the Therapy Manager.Adhere to Morningside Core Anchors and Good to GREAT Customer Service model.

Adhere to standard precautions.Use personal protective equipment when being exposed to bodily fluids.
Other:
Complete other work duties as assigned. Such assigned duties will not fall outside the therapist's scope of practice and will not violate existing policies and procedures, laws, or regulations.
Qualifications:
Education:
Requires a graduate degree from an APTA accredited Physical Therapy Program or equivalent for foreign trained Therapists.Certifications/Licenses:
Current Texas State License as a Physical T Current Driver's License. Current BLS.Training and Experience: Physical Therapists must demonstrate and exercise the knowledge and skills necessary to provide quality care appropriate to the age of the residents served.
