Position Summary:

Under the direction of the Clinical Coordinator, the Clinical Pharmacist assumes responsibility for patients’ pharmacotherapy across the continuum of the medication use process including the filling and dispensing of medication orders; ensuring the proper handling, usage and storage of drugs in the Pharmacy, maintaining standards set by the department, and translating policies and procedures into the daily operation of the Pharmacy. Responsible for acting as an information resource to patients, physicians, nurses and other health care professionals regarding medication therapy and drug related information and literature. Ability to plan and organize work schedule of support personnel as required.

Knowledge of State and Federal regulations governing drug distribution.

Complete knowledge of pharmacology and therapeutics.

Knowledge of sterile and aseptic procedures, compatibilities, drug interactions, drug therapy, toxicology and dosing parameters.

Knowledge of literature and reference sources.

Ability to instruct others.

Must be able to review medication therapies for appropriate dose for patients from neonates to geriatrics. Knowledge of pharmacokinetic dosing, AHA/ACC Guidelines, IDS Guidelines, ACCP Guidelines, medication reconciliation, nutritional support, and adverse drug reaction reporting is required.

Assumes the responsibilities of the Pharmacy Supervisor in the absence of the Clinical Coordinator, Supervisor, and Director.

Supervises Pharmacy Technicians and coordinates the Department's distribution functions

It is reasonable to anticipate that the incumbent will have occupational exposure to bloodborne pathogens in the course of his/her regularly scheduled activities as an associate of Brockton Hospital.

BASIC KNOWLEDGE/SKILLS/APTITUDE/EXPERIENCE:

Ability to solve practical problems and deal with a variety of variables in situations where only limited standardization may exist.

Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

Must be able to reasonably make appropriate judgment in communications and actions with patients, physicians, other associates, outside agencies, and vendors.

Education/Experience/Licenses/Technical/Other:

Education: Bachelor of Science Degree in Pharmacy required. Doctor of Pharmacy or Bachelor of Science with Residency, or equivalent experience required.

Experience (Type & Length): Two years’ experience as a Registered Pharmacist in a hospital setting preferred

Certification/Licensure: Must be a licensed pharmacist in the State of Massachusetts.
