Position Summary:
The Nurse Practitioner is responsible for making face-to-face assessments of hospice patients in a variety of settings under all levels of hospice care. The NP will coordinate with the Physician and collaborate with team members to assess client eligibility for hospice and provide interventions as required for pain and symptom management while demonstrating expertise in patient care. This role will operate under the direct supervision of the Hospice Physician or the Hospice Medical Director.
Required Experience:Master's Degree in nursingCertified/licensed as a Nurse Practitioner in state in which practicingDEA License in State in which practicingIn good standing/able to participate in the Medicare & Medicaid programsMinimum of 5 years nursing experience preferred; clinical nursing care in oncology, hospice, geriatrics, intensive care preferred
Responsibilities:Accountable for an assigned caseload of patients to a developed plan of care.Adheres to privacy and security standards provided by all federal, state, and local authorities of the Health Insurance Portability and Accountability Act (HIPAA) and other applicable state laws.Provides care, treatment, and services in accordance with professional standards of practice, practice guidelines, law, and regulations.Develops and implements a comprehensive, goal focused, patient-centered care plan for each patient and makes updates in accordance with written orders from the physician ensuring patient collaboration and understanding.Makes referrals to other services as indicated.Prepares and maintains clinical documentation according to Agency policies and acceptable nursing standards.Performs other duties as assigned.
