Job DescriptionThe Case Manager is an individual licensed as a registered nurse or licensed practical nurse by the State Board of Nursing and employed by a nursing facility, and is responsible for facilitating safe discharges from a clinical perspective, updating managed care companies for continued stay of skilled nursing residents, distributing and communicating notice of medicare non-coverage forms, appeals, and reconsiderations.In this role, the Case Manager will:
Complete a holistic initial assessment of resident to determine discharge needs.Regularly re-evaluates resident nursing needs.Monitor the plan of care and makes necessary revisions as resident status and needs change.Initiate appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician when indicated, and in collaboration with the nurses assigned to the patient.Counsel the resident and family in meeting nursing related needs and provide routine updates to the resident and family.Provides health care instructions to the resident as appropriate per assessment and plan of care.Works with the interdisciplinary team to assess resident needs for training and discharge planning, analyze care areas, and develop and revise a comprehensive care plan and ensure that compliance is maintained with state and federal guidelines.Ensure that arrangements for equipment and other clinically necessary items and services are communicated to social services.The ideal candidate will have:
Current nursing licensure in state, CPR certification1+ year of nursing experience preferred.Minimum of 1-2 years' experience with geriatric clients.PDPM experience / financial reimbursement experience preferred.Use good judgement and make sound, independent decisions.Clinical knowledge and technical expertise in nursing.Solid understanding of data collection and analysis, including ability to understand financial reimbursement processes.Effective time management, with ability to organize and prioritize work.Patience, understanding and interpersonal skills.Ability to maintain confidentiality and handle sensitive information.Ability to work under pressure and to meet objectives and deadlines.Excellent communication skills (written and verbal, English).Excellent observation, verbal and written communication skills, problem solving skills, basic computer skills, and nursing skills per competency checklist.Ability to enlist cooperation of unit personnel and staff in other departments.
