Position Summary:We have an exciting opportunity to join our team as a Program Manager, Pharmacotherapy - **Cancer Center**.
In this role, the successful candidate will be responsible for creating and managing a specialized pharmacotherapy clinical program within an oncology team Work under minimal supervision to troubleshoot pharmacotherapy challenges and clinical program management Supervision over all pharmacotherapy needs in a clinical program Responsible for creating implementing and managing a clinical pharmacotherapy program with particular focus on participation in a collaborative drug therapy management CDTM agreement
Job Responsibilities:
1 Manage a clinical pharmacotherapy program within an oncology team via participation in multidisciplinary clinic rounds pharmacotherapeutic/pharmacokinetic/pharmacoeconomic consultation and facilitating transitions of care as needed Provider patient and caregiver medication education counseling Participate in educational initiatives2 Develop implement and manage a CDTM agreement with physician providers in assigned hematology/oncology patient care setting in accordance with state and institutional guidelines nbsp;3 Evaluates and reports measures of patient encounter productivity and identifies methods to enhance efficiency of such encounters4 Proficiency in USP Chapter lt;797 gt; compounding procedures for emergent clinic based compounding5 Active participation in Tumor Boards for select hematology/oncology clinical practice areas6 Provides oversight for quality assurance of clinical pharmacy programs to improve quality of care Particular focus on creation review and management of oncology Epic treatment plans and treatment pathways7 Participate and manage patients within an Ambulatory Antimicrobial Stewardship program8 Contribute to professional development of staff including Pharmacists and Clinical Pharmacists9 Create and manage practice guidelines to enhance pharmaceutical care and medication safety for hematology/oncology patients10 Clinical intervention adverse drug reaction and medication error documentation and reporting11 Enterprise committee involvement including Pharmacy amp; Therapeutics Medication Utilization Evaluation Cancer Center Clinical Services and select hematology/oncology medical committees nbsp;12 Responsible for Pharmacy Resident precepting; Pharmacy student mentoring; and Scholarly activity13 Identifies and provides expertise for Medication Use Evaluations and monograph authoring for P amp;T requests nbsp;14 Maintains professional competency and certification
Minimum Qualifications:To qualify you must have a Doctor of Pharmacy degree from an ACPE accredited school of pharmacy New York State Pharmacist Licensure with current NYS registration and CDTM certification or at least eligible to gain within 90 days of start date PGY1 pharmacy residency and PGY2 pharmacy residency training with at least 2 years specialty clinical pharmacy practice experience after PGY2 completion Board certification in Oncology pharmacy BCOP Self motivation good communication organizational and interpersonal skills.
Required Licenses:
C-Collaborative Drug Thrpy Mgt, Pharmacist License-NYS
Preferred Qualifications:2 or more years of pharmacy management experience Experience working in an antimicrobial stewardship program

NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $176,001.74 - $196,331.96 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
