Responsibilities Provides a wide range of nursing services including conducting the initial evaluation visit regular re-evaluations initiating the plan of care with necessary revisions and specialized care to adult and geriatric patients in their home. Specialized care may include preventive rehabilitative or maintenance therapies to patients based on their diagnosis and condition which represent the full spectrum including: chronic conditions of congestive heart failure coronary artery disease chronic obstructive pulmonary disease diabetes mellitus multiple sclerosis etc.; acute conditions such as stasis ulcers open surgical wounds infusion therapies required due to various conditions pneumonia etc.; and situations where case management and supervision by a skilled nurse is required. Assures compliance with established care plan following accepted nursing standards Mercy Medical Center Merced policies and procedures and physicians orders. Provides immediate direction to assigned LVNs and Certified Home Health Aides. May be required to take call. Functions independently to identify collect and develop a database of biophysical environmental self-care educational emotional and discharge planning needs for adult geriatric patients in order to establish priorities for patients in accordance with the patient's age disorder and ethnic background. Identifies those patients requiring specialized nursing care including preventative and rehabilitative care those requiring physical speech and occupational therapy services home health aide services and medical social services. Performs assessment functions including the patient's psychosocial physical functional and resource systems as required to assure competent nursing care provision. Plans and develops care plans utilizing the nursing process according to accepted national nursing practice standards. Demonstrates proficiency in comprehensive assessment with OASIS data set collection as required by Medicare Condition of Participation. Demonstrates a working knowledge of Title 22 Medicare Conditions of Participation and admission/discharge criteria for home health agency services. Qualifications Minimum: One year of Home Health nursing experience within the past 5 years. An exception will be considered if an applicant has one year of acute care Medical/Surgical Telemetry and/or ICU nursing experience within the last fiveyears. Only when adequate staffing allows. Must possess and maintain a current Registered Nurse license with the state of California. Must possess and maintain a current Basic Life Support (BLS) Certificate. Must possess and maintain a valid California Driver's license and proof of auto insurance. One year experience as a professional nurse with demonstrated clinical skills Must be able to drive a motor vehicle safely for moderate periods of time. Preferred: Public Health Home Health Nursing or Hospice experience. Knowledge of Medicare Medical private insurance and local community. BSN degree preferred. Second language skills in Spanish RN BLS drivers license car insurance. #RN@DH
