Job Summary:

Provides clinical expertise within the palliative care services team to improve quality of life and health for patients with complex chronic illnesses and approaching end-of-life illnesses.Builds effective relationships with referring physicians to ensure excellent team follow-up regarding patient consults and to build an improved culture of collaboration among specialists, primary care physicians, nurses, and other staff to meet the needs of palliative care patientsProactively identifies opportunities to improve the patient and family experience of care and to improve the efficiency and effectiveness of resourcesEnsure compliance with legal, regulatory, and clinical policies and proceduresEssential Duties and Responsibilities:

Provide consultation services in palliative careProvide education/guidance regarding the scope of Supportive Medicine/Palliative Care services to group physiciansMonitor and supervise Supportive Medicine home care delivery team(s)Facilitate clarification of patient and family goals of careDiagnosis, treatment, and management of acute and chronic healthconditions

Order, perform, and interpret laboratory and radiology testsPrescribe medications including controlled substancesOrder treatments and durable medical equipment as indicatedPerform other therapeutic or corrective measures as indicated, including urgent careConsult with primary care physician of services provided and collaborate with other physicians as neededAssist in all facets of care coordination for palliative care referralsPrepare and maintain accurate patient records, charts, and documents to support sound medical practice and reimbursement for services providedAssist in development of clinical practice guidelines/standards in support of quality palliative careTimely review, oversight, and documentation of patient care by team NP (s) or RNEducation of staffAdhere to the practice of confidentiality regarding patients, families, staff and theorganization

Home and Nursing home visits on occasionsWork with administrative team to improve operational efficiency of the Palliative Care sectionLicense and Certifications:

Doctor of MedicineBoard certified or Board Eligible in internal medicine or Palliative/Hospice MedicineCurrent Texas medical licenseExperience desired but not required.Kelsey Seybold Clinic accepts H1B Visas.
