Wound, Burn and Ostomy Nurse Practitioner

We seek a Nurse Practitioner / Physician Assistant with strong clinical wound care experience to help provide specialized wound care services to our post-acute patients and provide follow-up telehealth visits, as needed. As part of an interdisciplinary care team, this position will provide telehealth and onsite visits as part of ongoing medical management and acute interventions for patients in the post-acute care setting. We offer a position that showcases your strengths and provides a flexible schedule tailored to your availability.DUTIES AND RESPONSIBILITIES, may include but not be limited toEvaluate and manage all acute, chronic wounds, surgical wounds, and dermatologic woundsCoordinates the patient plan of care with the physician and the facility's clinical disciplines to address patient needs, evaluate the effectiveness of care, and assess changes to the patient medical condition to ensure healing outcome goalsChange dressings and/or perform medically necessary debridementEstablish plan of care and work within protocols established by the supervising physicianESSENTIAL ABILITIESSolid working knowledge of epidemiology, microbiology, and aseptic technique to include universal precautionsAbility to provide both telehealth and onsite visits to skilled nursing facilities.Must be able to commit to work a minimum of 10 hours per week, between the hours of 9am-5pm Monday through Friday, 2 or more hours time block each timeExcellent communication skills and ability to effectively communicate with patients, family members, and other medical professionals.QUALIFICATIONSMaster's degree in Nursing Licensure, Certification, and RegistrationCurrent, valid Florida APRN license Certification through the ANCC, AANP (or equivalent certification body)Current BLS required Medicare NPI #, 855I and 855r on file (Independent NP Medicare Provider Number/PTAN#)Must be able to commit to work a minimum of 10 hours per week, between the hours of 9am-5pm Monday through Friday.(in 2 or more hours time block each time)No weekends or evenings at this time.Wound care-focused clinical experience (3+ years); Wound Care Certification is desired.Geriatrics and telehealth experience are highly preferred.WE OFFERCompetitive base pay rateAdditional RVU-based incentive model as additional income potentialA supportive, collaborative environmentSchedules built around your availability and zoning consideration to help maximize patient visitsABOUT US:We are a collaborative team of medical professionals who are driven to revolutionize healthcare for the elderly.
